Parish Participation Form

If your son or daughter will be a 2024-25 student at Mount St. Mary or Bishop McGuinness, you will need a Parish Participation form signed to receive the participating Catholic tuition rate.

Please have your student personally hand their form Fr. Rex or Fr. Kalivela AFTER attending Saturday or Sunday Mass in the coming weeks. The form must be completed with the student’s name, parish name, 2024-25 grade level, family name and family address.

Fr. Rex or Fr. Kalivela will collect students’ forms after Masses and then those forms will be brought to the parish office, signed, recorded by Holly Smith and mailed directly to the high school.

Parents may not turn in the forms for their students. Fr. Rex and Fr. K want to put faces with names and be able - in good faith - to sign that your student is “participating” in Mass as a Christ the King parishioner.
