Pastor Affirmation

As the Pastor of the person named above, I state that the person is practicing the Catholic Faith and qualifies to serve as a Sponsor. 

I attest that this person is:

• A baptised Catholic in good standing with the Church, attends Mass and receives the sacraments regularly.
• If he/she is married, the marriage was blessed in the Catholic Church
• Practices their faith within a Catholic parish community, and lives a moral life.

I know that the above is true either because the candidate is personally known by me, or can be verified by the competent person in my parish office.  I place my e-signature below as the assurance that I know of no reason why the above signed person cannot be admitted to the role of Sponsor.
